Parent/Guardian Liability Release, Assumption of Risk, Indemnity, and Medical
Authorization Agreement
This Liability Release and Waiver of Claims (“Agreement”) is entered into between The Huddle Sports, LLC (“The Huddle Sports,” “Program,” “we,” or “us”) and the parent or legal guardian completing the accompanying registration form for the minor participant (“Participant” or “Child”). By signing this Agreement, the Parent/Guardian confirms that the information provided on the registration form is accurate and that the Parent/Guardian has legal authority to sign on behalf of the child.
1. Program Description and Scope
The Huddle Sports provides youth athletic training and after-school programming including recreational activities, competitions, educational instruction, and related services (“Activities”). Activities may include basketball training, games, drills, conditioning exercises, team-building activities, recreational play, and educational enrichment activities. This Agreement applies to all activities conducted by The Huddle Sports now or in the future.
2. Assumption of Risk
Participation in youth sports and recreational activities involves inherent risks including but not limited to slips, falls, collisions, sprains, fractures, concussions, illness, equipment malfunction, and other unforeseen events. The Parent/Guardian voluntarily assumes all known and unknown risks associated with participation.
3. Waiver and Release of Liability
In consideration for the participant being permitted to participate, the Parent/Guardian releases and discharges The Huddle Sports, LLC including its owners, employees, volunteers, contractors, affiliates, and facility owners from all claims, damages, or causes of action including those arising from negligence related to participation.
4. Indemnification
The Parent/Guardian agrees to defend, indemnify, and hold harmless The Huddle Sports from any claims, lawsuits, damages, or expenses including attorney’s fees arising from the participant’s involvement in the program.
5. Medical Authorization
The Parent/Guardian authorizes The Huddle Sports staff to provide first aid and seek emergency medical treatment if necessary. The Parent/Guardian accepts full financial responsibility for any medical expenses incurred.
6. Behavior, Dismissal and Financial Responsibility
Participants must follow all program rules and conduct standards. Bullying, violence, or disruptive behavior may result in dismissal from the program without refund. Parents remain responsible for all program fees and charges.
7. Photo and Media Release
The Parent/Guardian grants permission for the participant to be photographed or recorded during activities for promotional, educational, or marketing purposes unless otherwise indicated.
8. Dispute Resolution, Arbitration, and Class Action Waiver
Any dispute, claim, or controversy arising out of or relating to participation in The Huddle Sports program shall be resolved through binding arbitration rather than in court. The Parent/Guardian agrees that any claims must be brought individually and not as part of any class action or representative proceeding.
9. Legal Terms and Severability
If any portion of this Agreement is found invalid or unenforceable, the remaining provisions shall remain in full force and effect. This Agreement shall be governed by the laws of the applicable state.
Parent/Guardian Agreement and Signature
Acknowledgment of Understanding: The Parent/Guardian confirms they have carefully read this Agreement,
understand its contents, and voluntarily agree to its terms.
Electronic Signature Authorization: By signing electronically, the Parent/Guardian acknowledges that their electronic
signature is legally binding and equivalent to a handwritten signature.
